Ready to leave the hospital. Nowhere safe to go.

When a patient is medically ready for discharge but has nowhere safe to go, we give them a private room, three meals and staff on site around the clock, minutes from the hospital. You eliminate the avoidable days. The patient gets a home to recover in.

How it works

From medically ready to discharged, the same day.

Under DRG payment the hospital is paid per stay, not per day. Every day past the expected length of stay, while a patient waits on a skilled nursing bed, home equipment or a safe address, is an unreimbursed avoidable day, and an admitted patient boarding in the emergency department waits for that bed. We take the barrier to discharge off your hands.

01

Case management refers

A patient is medically ready but has a barrier to discharge. Care management confirms they meet our admission criteria; we confirm a bed the same day.

02

We house and support

A private bedroom, three meals, linens, daily check-ins, staff awake overnight and a wheelchair van for appointments.

03

Clinicians visit

Nursing and therapy come from licensed home health agencies, ordered by the discharging physician, just as they would at home.

04

Safe transition

Days to a few weeks later, once the barrier clears: to SNF or rehab, home with services, or permanent housing. A take-back agreement covers any change in condition.

The stay

A real home, a short walk from the hospital.

Furnished apartments in residential buildings, not a ward and not a shelter. Calm enough to recover in, close enough that the hospital is never far away.

  • A private bedroom for every patient, never more than three to an apartment.
  • Staff on site 24 hours a day, with a program director and care coordinator on call.
  • Three meals a day, fresh linens and cleaning, all included.
  • Rides to follow-up appointments in our own wheelchair-accessible van.
For hospitals and health systems

Fewer avoidable days. Better throughput. Your patient safe for as long as it takes.

A post-acute partner for your hard-to-place discharges: one per-diem contract, paid only for occupied beds, at a fraction of what an avoidable day costs you.

Reserved beds

A guaranteed number of beds held for your complex discharges, available same day, so care management always has a discharge option when the barrier is housing or caregiving.

Simple to buy

A flat nightly rate per occupied bed, invoiced monthly. No capital, no leases, no new staff on your side. We open in months, not years.

Measured every month

Avoidable days eliminated, length of stay against expected (GMLOS), 30-day readmissions and incidents, reported monthly so the value is visible to finance, capacity management and the floor.

We are housing with support services, not a licensed healthcare facility. Our staff do not provide hands-on medical care; that stays with your physicians and the home health agencies they order. Written admission criteria and a take-back agreement come with every contract.

Talk to us
